You have the right to copies of medical records in the possession of health care providers. This does not include notations and comments from practitioners that might be located within those records. To obtain a copy of any medical records, you or your representative need to request the copy in writing. Patients or their representatives are not to be charged for inspection of the medical record, but reasonable charges may be levied for copies. Regulations define reasonable charges for copying as not more than $12 for the first ten or fewer pages, and $.25 for each additional page. Actual postage costs may also be charged. You may be required to pay the actual cost of reproducing x-rays or other items which cannot be reproduced without special equipment.
