Public Access to Physician Information (Docinfo.org) Since 1912, the Federation of State Medical Boards has collected information on disciplinary actions taken against physicians, making our database the most sophisticated and trusted source for complete and accurate physician disciplinary information in the United States. Our comprehensive database contains information on disciplinary actions reported against physicians by regulatory and licensing entities. Those reporting entities include state medical allopathic and osteopathic boards, the Department of Health and Human Services, the United States Department of Defense, Educational Commission for Foreign Medical Graduates, Veterans Administration, National Board of Medical Examiners, Drug Enforcement Administration, and international licensing authorities currently including Canada, England, Australia and New Zealand.

To learn if your physician has any disciplinary history, please print out the Disciplinary Search Request Form from your computer and mail to us. You must include the physician's first and last name, and the address, city, state and zip code he or she is located in. Please be sure to include your mailing address, along with your Master Card or Visa credit card information. You may also pay by check by making checks payable to the Federation of State Medical Boards. When paying by check, please attach your check to the Physician Profile Request Form.
