I think the belle-Brown Pain Scale is a real good one as far as how I rate my pain!

The Pain Map Worksheet with drawings of body is excellent....and to think I took digital pictures of my body and drew arrows with comments where all my pain is and how it feels. This sure would have simplified it!!

Randall Pain Scale Reporting Form is also very good.

My doctor thought my body photos etc. were excellent, wait till he sees me next time (LOL)! JEAN
