Rose,
I also printed a card with all my surgeries listed on, date, doctors and
hospitals. I then laminated it.
When they ask for that info. I just give them the card and they make a copy
of it. I have gotten lots of compliments about it from the nurses and
doctors.
I also did one for my dad of his meds.
