Dear Beth,
You need to find out what codes your insurance
will pay on and ask one of the insurance operators
which ones your policey accepts.I don't know the
exact codes but I do know only certain codes some
insurance plans except.If you dont' get help the
first time you call you'r insurance carrier call
back every time you call you get someone diffrent
and some are more helpful than others.
