Medication for pain?

I've been on everything there is over the past several years. I've tried Darvocet N-100, Vicodan, Ultram, Demerol tablets, MS Contin, Demerol IV, Morphine IV, Duragesic, and Actiq (Fentanyl 600 mcg). Right now, I'm on Duragesic .75mg patch every three days and Actiq 600 mcgs every six hours prn breakthrough pain. It's horrible to be addicted, but I'm in the ER all the time if I don't keep the pain controlled. Even with this heavy medication regime, I still end up in the ER frequently with obstructions, vomiting, dehydration, etc. I've heard that the Vicodan (Lortab) causes really bad constipation (so do all opiates) but, if you're obstructed or having pencil-thin stools anyway, what difference does it make!?
