Is physical therapy harmful for someone w/adhesions?

I had extremely major surgery on my right lung in February. I have been improving and am back to work, but the pain level I am experiencing constantly has stayed about the same for the past three months. Physical activity of any kind worsens it. After seeing my doctor Fri. he mentioned that I might have adhesions, I am scheduled to begin physical therapy, mainly to improve my shoulder, but I think the problem is actually in my right ribcage. Is there the possibility that stretching during therapy might further damage tissue or any organs? I am counting on it being very painful, so I am prepared for that, but I would hate to think that I could do further damage and have to undergo surgery again. Is there any kind of medication that will dissolve these and how do you find out for sure you have them? I do have a past history of scar tissue around my female organs, but no current problems in that area. I hope someone can help. Thanks.
