If you are having bowel movements, you do not have an obstruction. And
yes, you almost always vomit with an obstruction. And I think gas/food
moving through the intestines is a major cause of pain from adhesions.
If you think of the food snaking through the intestines normally, they
are free to move around as they please. If you have a piece of
intestine adhesed somewhere, it can create unnatural 'kinks' and narrow
spots that slow down passage of the gas/food. This can be very,very
painful.

Now I got to get back to life before someone thinks I'm a bad doctor. :
